  • Your full legal name.
  • A valid email address.
  • Access to the internet.


  1. Go to and enter the time and date of your visit and your “entry ID”. All of this information is located on your Kroger store receipt.
  2. Select the specific departments within the store from which you made the purchases on your receipt.
  3. Rate your experience in the establishment while shopping.
  4. Answer questions pertaining to the method of checkout.
  5. Give your contact information such as name, and email address and phone number.
  6. Provide personal information such as gender and age to complete the survey and be entered into the sweepstakes drawing.
